At present, the problem of the spinal failed surgeries is one of the biggest challenges that surgeons face. In the last decades the number of spinal surgeries has increased exponentially in our country and overseas. Unfortunately it means a higher and more frequent presence of patients with bad clinical results after surgery.

Roughly a 10-20% of the patients operated will need a new surgery in the forthcoming10 years. We are able to know the reason of bad results in a 90% of the surgeries. Therefore we will know what caused a bad result in a  90% of the cases and deal with it.

The most common reasons of bad results after spinal surgeries are:

• Foraminal stenosis  25-29%
• Pain caused by a disc  20-22%
• Non-union of bony fractures (Pseudarthrosis) 14%
• Neuropathic pain  10%
• Recurring herniated disc  7-12%
• Facet pain  3%
• Sacroiliac pain  2%

The evaluation of a patient, who has undergone a previous failed surgery, must be specially thorough. That way  we can get an interrelationship between the symptons of the patient and the findings of tests. This evaluation will allow us to decide what patients would benefit from a re-operation as opposed to those who would need another kind of treatments.

We can gather the instructions for re-operation in the following ones:

• Recurring stenosis in a compressed level
• New stenosis in an adjoining level to an already existing one
• Scanty liberation in the previous surgery
• Bad-positioned instrumentation
